About The Role | He kōrero whakamārama mō tēnei tūranga
This is a part-time permanent role working 16 hours per week. No shift work or evenings!
This role will be based across the Tairāwhiti region. You must be comfortable travelling across this area.
As a Clinical Coordinator, the successful candidate will be responsible for introducing Geneva Healthcare and obtaining consent to plan and provide Home and Community Support Services to support and empower our clients and whanau. You will be implementing initial Service Plans and ongoing service plan reviews based on funder requirements and when the client has changes in their health status or care requirements.
